  • Osteoporosis Treatment

    Mary, a 60-year-old woman, was seen in the office for Osteoporosis treatment. She had a recent bone density test as she was concerned about having stopped HRT and the risk to her bones. Her BMD was lower than normal and according to Osteoporosis Guidelines; she was assessed as being at moderate risk. Since at that level, she may or may not be treated; further history was needed.

    Had she fallen more than twice in the last year? Did she fracture any bones (other than face and fingers) since age 40 from a less than traumatic incident? What drugs, if any, is she taking on a regular basis? Has she lost height? These are important issues to discuss with her doctor for osteoporosis treatment . Your pharmacist can review your medication and alert you to any drugs that affect bone. Your dietitian can help you increase your calcium-containing foods. And your physiotherapist or trainer can help you focus on the correct exercise to maintain good posture and decrease the risk of falling. Whether or not specific medication is needed depends on many variables and you need to control the lifestyle options to prevent deterioration.

    Trudy is a 54-year-old woman who comes to see her family doctor with ongoing issues of poor sleep, hot flashes, night sweats and increasing irritability. Her last period was 2 years earlier and she has tried a number of over-the-counter remedies, such as evening primrose oil and red clover. These have had minimal benefit. She has been reluctant to consider hormones, worrying about the side effects. She is otherwise healthy, a non-smoker, and active when not so tired and anxious. She is particularly upset as her spouse is becoming affected by her mood and discomfort and she is aware that their quality of life, both personally and as a couple, has deteriorated. Her symptoms have become disabling.

    One of the first recommendations is for Trudy to seek help from her pharmacist. Likely she can try some options for sleep, such as valerian, and may get some relief. It is important to utilize lifestyle options such as dressing in layers, considering physiotherapy to improve flexibility and general activity levels and avoiding triggers, if possible.

    A new option for hot flashes is the Canadian-made, “Menopod”, a cooling handheld device designed to quickly lower body temperature. Trudy should also meet with her doctor or primary care provider to consider prescription medication.

  • Treatment of Vaginal Atrophy " Karen a 60-year-old woman with recurrent urinary infections, vaginal dryness and itch"

    Karen is a 60-year-old woman who comes into her physician's office with recurrent urinary infections, vaginal dryness and itch. She has been using anti-yeast medication repeatedly and despite antibiotics, the bladder infections recur. On exam the genital area is raw, pale and easily tears with contact. Her past medical history is unremarkable, with her last menstrual period about five years ago. These symptoms have been getting worse with time and she no longer can comfortably have intercourse with her husband.

    Karen has been very reluctant to discuss these issues as she feels uncomfortable and doesn’t want to complain. However, the urinary issues are just getting worse.

    Given the amount of over-the-counter medication she has purchased and the antibiotics she has received at various walk-in clinics, her pharmacist has raised the issue of the estrogen deficiency in the genitourinary area and suggested she see her doctor. Armed with the support of the pharmacist, she feels a bit better going to see the doctor to discuss these issues.
